**About the Role** In this role you will: * Support residents in our Community Home in Clouston Park, Upper Hutt to lead engaging and fulfilling lives * Follow care plans that enable residents in their health and wellbeing * Organise daily activities for residents and take residents out to the community for activities, medical appointments, etc. * Work collaboratively with the Rehabilitation Team to support residents to achieve lifestyle goals and community connections * Continue to achieve NZQA Health and Wellbeing Certificate levels with support of your manager and Workforce Development team
